Carriers match these details against public and IRS records. A single mismatch is the most common reason a brand registration is rejected.

Leave blank if you only operate under the legal name. A DBA that appears in messages should be listed here.

Nine digits from the IRS. This is the primary key carriers use to verify the brand.

Must match the entity type on file with the IRS and your state. Sole proprietors follow a different, more limited registration path.
